ABSTRACT

ABSTRACT Objectives: Apoptosis effect of oral alpha-blockers is known in the prostate. Apoptosis index of silodosin has not been proved, yet. Aims are to present apoptosis index of silodosin in prostate and to compare this with other currently used alpha-blocker's apoptosis indexes together with their clinical effects. Materials and Methods: Benign prostatic hyperplasia (BPH) patients were enrolled among those admitted to urology outpatient clinic between June 2014 and June 2015. Study groups were created according to randomly prescribed oral alpha-blocker drugs as silodosin 8mg (Group 1; n=24), tamsulosin 0.4mg (Group 2; n=30), alfuzosin 10mg (Group 3; n=25), doxazosin 8mg (Group 4; n=22), terazosin 5mg (Group 5; n=15). Pa- tients who refused to use any alpha-blocker drug were included into Group 6 as control group (n=16). We investigated apoptosis indexes of the drugs in prostatic tissues that were taken from patient's surgery (transurethral resection of prostate) and/or prostate biopsies. Immunochemical dyeing, light microscope, and Image Processing and Analy- sis in Java were used for evaluations. Statistical significant p was p<0.05. Results: There were 132 patients with mean follow-up of 4.2±2.1 months. Pathologist researched randomly selected 10 areas in each microscope set. Group 1 showed statisti- cal significant difference apoptosis index in immunochemical TUNEL dyeing and im- age software (p<0.001). Moreover, we determined superior significant development in parameters as uroflowmetry, quality of life scores, and international prostate symptom score in Group 1. Conclusions: Silodosin has higher apoptosis effect than other alpha-blockers in prostate. Thus, clinic improvement with silodosin was proved by histologic studies. Besides, static factor of BPH may be overcome with creating apoptosis.

ABSTRACT Background Nonsteroidal anti-inflammatory drugs induces gastric mucosal lesions because of its acidic properties. Ranitidine, an H2 receptor antagonist, has proved beneficial in patients with gastric ulcers. Objective The present study was performed to assess the effect of administering ranitidine in Nonsteroidal anti-inflammatory drugs (diclofenac, nimesulide) induced gastropathy, and their effect on the histopathology of stomach, kidney and liver. Methods Diclofenac, nimesulide, and ranitidine were administered in doses of 2, 4, and 6 mg/kg, p.o. once daily for 14 days, and their effect on gastric volume, acidity, mean ulcer number, and gastric pH. In addition, histopathological examination was also performed on sections of stomach, kidney and liver. Results Following the administration of diclofenac or nimesulide, all the gastric parameters were significantly altered as well as the histopathology of stomach, liver and kidney. In the control group, the renal sections showed normal glomeruli with no thickening of glomerular basement membrane, while in diclofenac alone, nimesulide alone, and ranitidine with nimesulide groups, the thickening of glomerular basement membrane was observed. These alterations were observed to be reversed in the ranitidine with diclofenac group. In the sections from the liver, the control group showed anastomosing plates and cords of cuboidal hepatocytes with round well stained nuclei and abundant cytoplasm. In the ranitidine with diclofenac, and ranitidine with nimesulide groups, mild dilatation of sinusoids is seen coupled with prominence of central vein. In the diclofenac alone and nimesulide alone groups, the proximal and distal convoluted tubules show mild focal tubular necrosis. In the gastric sections, the control group showed several folds forming villi, and the epithelial lining surface of the mucosa. In the ranitidine with diclofenac, and ranitidine with nimesulide groups, the duodenum showed scattered inflammatory cells composed predominantly of lymphocytes. In diclofenac alone and nimesulide alone group, the sections from the gastric areas showed partial necrosis and mild chronic inflammation respectively. Conclusion The study, therefore, has provided therapeutic rationale towards simultaneous administration of H2 receptor blocker ranitidine with diclofenac to be more beneficial as compared to ranitidine with nimesulide, to minimise the gastric intolerance of diclofenac in long term treatment of inflammatory conditions.


RESUMO Contexto Anti-inflamatórios não esteroidais induzem lesões da mucosa gástrica devido às suas propriedades ácidas. Ranitidina, um antagonista dos receptores H2, revelou-se benéfico em pacientes com úlceras gástricas. Objetivo - O presente estudo foi realizado para avaliar o efeito da administração de ranitidina em gastropatia induzida por anti-inflamatórios não esteroidais (diclofenaco, nimesulida) e seu efeito sobre a histopatologia do estômago, dos rins e fígado. Métodos Diclofenaco, nimesulida e ranitidina foram administradas em doses de 2, 4 e 6 mg/kg, p.o. uma vez diariamente por 14 dias e seu efeito sobre o volume gástrico, acidez, significam o número de úlcera e o pH gástrico. Além disso, o exame histopatológico também foi realizado em seções do estômago, dos rins e fígado. Resultados Após a administração de diclofenaco ou nimesulida, todos os parâmetros gástricos foram significativamente alterados assim como a histopatologia do estômago, fígado e rim. No grupo controle, as seções renais mostraram glomérulos normais sem espessamento da membrana basal glomerular, enquanto em diclofenaco isolado, nimesulida isolado e grupos com ranitidina e nimesulida, foi observado espessamento da membrana basal glomerular. Estas alterações observou-se serem revertidas no grupo ranitidina com diclofenaco. As seções do fígado, o grupo controle mostrou placas e cordões de hepatócitos cuboidais anastomosados com núcleos bem demarcados e citoplasma abundante. Nos grupos ranitidina com diclofenaco e ranitidina com nimesulida, leve dilatação dos sinusoides é vista acoplados com proeminência de veia central. Nos grupos diclofenaco e nimesulida sozinhos, túbulos proximais e distais contorcidos mostram necrose tubular focal leve. Nas secções gástricas, o grupo controle mostrou várias dobras formando vilosidades e a superfície do revestimento epitelial da mucosa. Nos grupos ranitidina com diclofenaco e ranitidina com nimesulida, o duodeno mostrou dispersas células inflamatórias predominantemente compostas por linfócitos. Nos grupos diclofenaco e nimesulida sozinhos, as secções de áreas gástricas mostraram necrose parcial e inflamação crônica moderada respectivamente. Conclusão - O estudo, portanto, forneceu o fundamento terapêutico para administração simultânea de bloqueador de receptor H2 (ranitidina) com diclofenaco, sendo mais benéfica em comparação com ranitidina com nimesulida para minimizar a intolerância gástrica de diclofenaco no tratamento a longo prazo de condições inflamatórias.

ABSTRACT

BACKGROUND: Computer-based technology is becoming increasingly essential in biological research where drug discovery programs start with the identification of suitable drug targets. 2-Methoxyestradiol (2ME2) is a 17ß-estradiol metabolite that induces apoptosis in various cancer cell lines including cervical cancer, breast cancer and multiple myeloma. Owing to 2ME2's poor in vivo bioavailability, our laboratory in silico-designed and subsequently synthesized a novel 2ME2 analogue, 2-ethyl-3-O-sulphamoyl-estra-1,3,5(10),15-tetraen-17-ol (ESE-15-ol), using receptor- and ligand molecular modeling. In this study, the biological effects of ESE-15-ol (180 nM) and its parent molecule, 2ME2 (1 µM), were assessed on morphology and apoptosis induction in cervical cancer cells. RESULTS: Transmission electron microscopy, scanning electron microscopy and polarization-optical transmitted light differential interference contrast (PlasDIC) images demonstrated morphological hallmarks of apoptosis including apoptotic bodies, shrunken cells, vacuoles, reduced cell density and cell debris. Flow cytometry analysis showed apoptosis induction by means of annexin V-FITC staining. Cell cycle analysis showed that ESE-15-ol exposure resulted in a statistically significant increase in the G2M phase (72%) compared to 2ME2 (19%). Apoptosis induction was more pronounced when cells were exposed to ESE-15-ol compared to 2ME2. Spectrophotometric analysis of caspase 8 activity demonstrated that 2ME2 and ESE-15-ol both induced caspase 8 activation by 2- and 1.7-fold respectively indicating the induction of the apoptosis. However, ESE-15-ol exerted all of the above-mentioned effects at a much lower pharmacological concentration (180 nM) compared to 2ME2 (1 µM physiological concentration). CONCLUSION: Computer-based technology is essential in drug discovery and together with in vitro studies for the evaluation of these in silico-designed compounds, drug development can be improved to be cost effective and time consuming. This study evaluated the anticancer potential of ESE-15-ol, an in silico-designed compound in vitro. Research demonstrated that ESE-15-ol exerts antiproliferative activity accompanied with apoptosis induction at a nanomolar concentration compared to the micromolar range required by 2ME2. This study is the first study to demonstrate the influence of ESE-15-ol on morphology, cell cycle progression and apoptosis induction in HeLa cells. In silico-design by means of receptor- and ligand molecular modeling is thus effective in improving compound bioavailability while preserving apoptotic activity in vitro.

ABSTRACT PURPOSE: To test whether hemorrhagic shock (HS) increases the Cyclooxygenase-2 (COX-2) expression in the intestine and whether this enhanced COX-2 expression mediates the intestinal dysmotility after HS. METHODS: Male Wistar rats were randomly divided into HS sham group and HS group. At 180 min following HS establishment, the duodenum samples were harvested to assess the motility function, protein expression of COX-2 and the downstream products of COX-2, prostaglandins. RESULTS: Examination of motility function ex vivo showed that the contractile response to acetylcholine of smooth muscle strips of rats subjected to HS was significantly suppressed. A COX-2 inhibitor, NS-398, abolished this depressed contractile response after HS. Western blotting revealed an increased protein expression of COX-2 in intestinal tissues of HS rats. Immunohistochemical examination indicated that intestine tissues of HS rats were manifested by part of villous expansion and disruption, a large amount of COX-2 positive cells appearance in lamina propria and submucosa. Furthermore, the contents of prostaglandin E2 was significantly increased in intestinal tissues of HS rats. CONCLUSION: The enhanced COX-2/ prostaglandin E2 involves in the hemorrhagic shock induced intestinal dysmotility.

OBJECTIVE: To assess the knowledge and practice of pediatricians about infants with physiological reflux and gastroesophageal reflux disease. METHODS: 140 pediatricians were interviewed during two scientific events in 2009 and 2010. The questions referred to two clinical cases of infants. One with symptoms of infant regurgitation (physiological reflux) and another with gastroesophageal reflux disease. RESULTS: Among 140 pediatricians, 11.4% (n=16) and 62.1% (n=87) would require investigation tests, respectively for infant regurgitation (physiological reflux) and gastroesophageal reflux disease. A series of upper gastrointestinal exams would be the first requested with a higher frequency. Medication would be prescribed by 18.6% (n=6) in the case of physiological reflux and 87.1% (n=122) in the case of gastroesophageal reflux disease. Prokinetic drugs would be prescribed more frequently than gastric acid secretion inhibitors. Sleeping position would be recommended by 94.2% (n=132) and 92.9% (n=130) of the respondents, respectively for the case of physiological reflux and gastroesophageal reflux disease; however, about half of the respondents would recommend the prone position. Only 10 (7.1%) of the pediatricians would exclude the cow's milk protein from the infants' diet. CONCLUSIONS: Approaches different from the international guidelines are often considered appropriate, especially when recommending a different position other than the supine and prescription of medication. In turn, the interviews enable us to infer the right capacity of the pediatricians to distinguish physiologic reflux and gastroesophageal reflux disease correctly. .


OBJETIVO: Avaliar o conhecimento e a prática de pediatras brasileiros na assistência ao lactente com refluxo fisiológico e doença do refluxo gastroesofágico. MÉTODOS: Foram entrevistados 140 médicos pediatras em dois eventos científicos em 2009 e 2010. As perguntas referiam-se a dois casos clínicos de lactentes, um com quadro compatível com regurgitação do lactente (refluxo fisiológico) e outro com doença do refluxo gastroesofágico. RESULTADOS: Dos 140 participantes, 11,4% (n=16) e 62,1% (n=87) solicitariam exame para lactentes, respectivamente, com refluxo fisiológico e doença do refluxo gastroesofágico. O primeiro exame solicitado com maior frequência seria a radiografia contrastada de esôfago, estômago e duodeno. Medicação seria prescrita por 18,6% (n=26) para o caso de refluxo fisiológico e 87,1% (n=122) para o caso de doença do refluxo gastroesofágico. Procinéticos seriam prescritos com maior frequência do que os redutores da secreção ácida gástrica. Prescrição de posição para dormir fez parte das recomendações de 94,2% (n=132) e 92,9% (n=130) dos entrevistados, respectivamente, para os casos de refluxo fisiológico e doença do refluxo gastroesofágico. Entretanto, cerca da metade dos entrevistados não recomendaria o decúbito dorsal. Prescrição de dieta de exclusão do leite de vaca para um lactente com quadro de doença do refluxo gastroesofágico seria feita por apenas 10 (7,1%) dos participantes. CONCLUSÕES: Condutas diferentes das diretrizes internacionais são frequentemente consideradas adequadas, especialmente quanto à recomendação de posição diferente do decúbito dorsal e prescrição de medicamentos. As respostas permitem inferir a capacidade de correta diferenciação entre refluxo fisiológico e doença do refluxo gastroesofágico. .

OBJECTIVE: Vitamin D deficiency (VDD) in pregnant women and their children is an important health problem with severe consequences for the health of both. Thus, the objectives of this review were to reassess the magnitude and consequences of VDD during pregnancy, lactation and infancy, associated risk factors, prevention methods, and to explore epigenetic mechanisms in early fetal life capable of explaining many of the non-skeletal benefits of vitamin D (ViD). DATA SOURCE: Original and review articles, and consensus documents with elevated level of evidence for VDD-related clinical decisions on the health of pregnant women and their children, as well as articles on the influence of ViD on epigenetic mechanisms of fetal programming of chronic diseases in adulthood were selected among articles published on PubMed over the last 20 years, using the search term VitD status, in combination with Pregnancy, Offspring health, Child outcomes, and Programming. DATA SYNTHESIS: The following items were analyzed: ViD physiology and metabolism, risk factors for VDD and implications in pregnancy, lactation and infancy, concentration cutoff to define VDD, the variability of methods for VDD detection, recommendations on ViD replacement in pregnant women, the newborn and the child, and the epigenetic influence of ViD. CONCLUSIONS: VDD is a common condition among high-risk pregnant women and their children. The routine monitoring of serum 25(OH)D3 levels in antenatal period is mandatory. Early preventive measures should be taken at the slightest suspicion of VDD in pregnant women, to reduce morbidity during pregnancy and lactation, as well as its subsequent impact on the fetus, the newborn and the child. .


OBJETIVO: Deficiência de vitamina D (DVD) nas gestantes e seus filhos é problema de saúde, com consequências graves à saúde de ambos. Assim, esta revisão visou reavaliar a magnitude e as consequências da DVD na gestação, lactação e infância, fatores de risco associados, métodos de prevenção, além de explorar os mecanismos epigenéticos na vida fetal capazes de explicar benefícios não-esqueléticos da vitamina D (ViD). FONTES DE DADOS: Selecionaram-se artigos originais, de revisão e consensos com nível elevado de evidência para decisões clínicas relacionadas à DVD na saúde das gestantes e seus filhos e artigos sobre sua ação sobre os mecanismos epigenéticos da programação fetal de doenças crônicas na vida adulta, publicados no PubMed nos últimos 20 anos, empregando-se VitD status, e em combinação com Pregnancy, Offspring health, Child outcomes e Programming. SÍNTESE DOS DADOS: Abordou-se fisiologia, metabolismo, fatores de risco para a DVD e implicações na gravidez, lactação e infância, concentração de corte para definir DVD, variabilidade de métodos na sua detecção, recomendações sobre a reposição de ViD nas gestantes, no recém-nascido e na criança, bem como sobre ter as influências epigenéticas da ViD. CONCLUSÕES: DVD é frequente entre gestantes de alto risco e seus filhos. Monitorar rotineiramente os níveis séricos de 25(OH)D3 no período antenatal é imperativo. Medidas preventivas precoces devem ser instituídas à menor suspeita de DVD na gestante, para reduzir morbidades durante a gestação e a lactação, bem como seu posterior impacto sobre o feto, o recém-nascido e na infância. .

Os anti-inflamatórios não esteroidais (AINEs) são medicamentos utilizados no alívio da dor após a ativação dos aparelhos ortodônticos, mas estas substâncias podem influenciar a formação óssea ou remodelação. Diante da possibilidade de interferência dos medicamentos durante o tratamento ortodôntico, foi avaliado o efeito á curto prazo de AINEs e anti-inflamatório seletivo COX-2, em doses terapêuticas, sobre osteoblastos durante a movimentação dentária induzida. Os fármacos foram determinados através de questionários aplicados a ortodontistas, os quais mais selecionaram os mais prescritos para alívio da dor durante o tratamento ortodôntico. Os medicamentos selecionados e a nimesulida (seletivo COX-2) foram administrados em uma amostra de 80 ratos albinos da linhagem Wistar, nos quais foi realizada a instalação de dispositivos constituídos por uma mola de secção fechada ancorada aos incisivos centrais superiores, movimentando mesialmente o primeiro molar superior esquerdo. Os animais foram distribuídos em quatro grupos de 20 de acordo com a administração medicamentosa diária: paracetamol, ibuprofeno, nimesulida e um grupo controle (animais não medicados). E divididos em subgrupos de 5 de acordo com o tempo de tratamento da movimentação dentária induzida: 3, 5 e 7 dias. Posteriormente, os animais receberam doses letais da mistura de relaxante muscular e anestésico por via intramuscular para coleta do material, o qual foi devidamente processado, corado com hematoxilina-eosina e submetido à análise microscópica óptica para avaliar a quantidade de osteoblastos, na área de tensão, do osso adjacente de cada raiz distovestibular dos primeiros molares superiores esquerdo. Os resultados mostraram que o uso de paracetamol até 5 dias pode gerar interferências na formação óssea, pois diminuiu o número de osteoblastos e que o ibuprofeno foi a droga que melhor agiu por apresentar menor ação de inibição sobre os osteoblastos num período de uso de até...


The nonsteroidal anti-inflammatory drugs (NSAIDs) are drugs used to relieve pain after activation of orthodontic appliances, but these substances can influence bone remodeling and formation. Faced with the possibility of interference of drugs in treatments, the effects will be short-term NSAIDs and COX-2 selective antiinflammatory in therapeutic doses on osteoblasts during induced tooth movement. The drugs were determined through questionnaires given to orthodontists, selecting then, the most commonly prescribed for pain relief during orthodontic treatment. The selected drugs and nimesulide (selective COX-2) were administered in a sample of 80 albino Wistar rats, in which the installation of devices consisted of an enclosed section spring anchored to the upper central incisors, moving out mesially the first upper left molar. The animals were divided into four groups of 20 according to the daily drug administration: acetaminophen, ibuprofen, nimesulide and a control group (animals not treated). Then, divided into subgroups of 5 according to the treatment time of the induced tooth movement, 3, 5 and 7 days. Subsequently, the animals received lethal doses of the mixture of anesthetic and muscle relaxant intramuscularly for the collection of the material, which has been properly processed, stained with hematoxylin-eosin and subjected to microscopic analysis to assess the amount of osteoblasts in the stressed area of the adjacent bone of each distobuccal root of the first left molars. The results showed that the use of acetaminophen up to 5 days will cause interference in bone formation decreasing the number of osteoblasts and ibuprofen was the drug that best acted by having less inhibiting action on osteoblasts in a usage period of up to 7 days. It is suggested that the ideal to relieve pain and/or discomfort caused by orthodontic movement without prejudice to the bone repair would be the use of the associated medication. ABSTRACT

Os anti-inflamatórios não esteroidais (AINEs) são medicamentos utilizados no alívio da dor após a ativação dos aparelhos ortodônticos, mas estas substâncias podem influenciar a formação óssea ou remodelação. Diante da possibilidade de interferência dos medicamentos durante o tratamento ortodôntico, foi avaliado o efeito á curto prazo de AINEs e anti-inflamatório seletivo COX-2, em doses terapêuticas, sobre osteoblastos durante a movimentação dentária induzida. Os fármacos foram determinados através de questionários aplicados a ortodontistas, os quais mais selecionaram os mais prescritos para alívio da dor durante o tratamento ortodôntico. Os medicamentos selecionados e a nimesulida (seletivo COX-2) foram administrados em uma amostra de 80 ratos albinos da linhagem Wistar, nos quais foi realizada a instalação de dispositivos constituídos por uma mola de secção fechada ancorada aos incisivos centrais superiores, movimentando mesialmente o primeiro molar superior esquerdo. Os animais foram distribuídos em quatro grupos de 20 de acordo com a administração medicamentosa diária: paracetamol, ibuprofeno, nimesulida e um grupo controle (animais não medicados). E divididos em subgrupos de 5 de acordo com o tempo de tratamento da movimentação dentária induzida: 3, 5 e 7 dias. Posteriormente, os animais receberam doses letais da mistura de relaxante muscular e anestésico por via intramuscular para coleta do material, o qual foi devidamente processado, corado com hematoxilina-eosina e submetido à análise microscópica óptica para avaliar a quantidade de osteoblastos, na área de tensão, do osso adjacente de cada raiz distovestibular dos primeiros molares superiores esquerdo. The nonsteroidal anti-inflammatory drugs (NSAIDs) are drugs used to relieve pain after activation of orthodontic appliances, but these substances can influence bone remodeling and formation. Faced with the possibility of interference of drugs in treatments, the effects will be short-term NSAIDs and COX-2 selective antiinflammatory in therapeutic doses on osteoblasts during induced tooth movement. The drugs were determined through questionnaires given to orthodontists, selecting then, the most commonly prescribed for pain relief during orthodontic treatment. The selected drugs and nimesulide (selective COX-2) were administered in a sample of 80 albino Wistar rats, in which the installation of devices consisted of an enclosed section spring anchored to the upper central incisors, moving out mesially the first upper left molar. The animals were divided into four groups of 20 according to the daily drug administration: acetaminophen, ibuprofen, nimesulide and a control group (animals not treated). Then, divided into subgroups of 5 according to the treatment time of the induced tooth movement, 3, 5 and 7 days. Subsequently, the animals received lethal doses of the mixture of anesthetic and muscle relaxant intramuscularly for the collection of the material, which has been properly processed, stained with hematoxylin-eosin and subjected to microscopic analysis to assess the amount of osteoblasts in the stressed area of the adjacent bone of each distobuccal root of the first left molars. The results showed that the use of acetaminophen up to 5 days will cause interference in bone formation decreasing the number of osteoblasts and ibuprofen was the drug that best acted by having less inhibiting action on osteoblasts in a usage period of up to 7 days. ABSTRACT

PURPOSE: To evaluate the genotoxicity of propolis and L-lysine, as well as their effects on the possible cellular damage in erythroblasts (bone marrow) and leukocytes (peripheral blood) caused by the carcinogen BBN (n - butyl - n {4 - hydroxybutyl} nitrosamine) in rats subjected to bladder carcinogenesis and treated with green propolis and L-lysine. METHODS: One hundred and twenty five rats were distributed into the following groups: I, IIA, IIB, III, K, L M N, X, XI, XII and XIII. Groups I to X received BBN in drinking water for 14 weeks (wks). Group I was treated with intragastric (ig) propolis at 150 mg/kg body weight, for 44 wks, beginning 30 days before start of BBN. Groups IIA and III were treated with propolis (150 mg/kg), for 40 wks, subcutaneous (sc) and ig, respectively, beginning simultaneously with BBN. On the 32nd wk, the animals of groups L, M and N were treated ig with L-lysine (300 mg/kg), celecoxib (30 mg/kg) and propolis (300 mg/kg), respectively, up to the 40th wk. The groups that received only BBN (IIB and K) were treated with water, sc and orally, respectively, for 40 wks. Groups XI, XII and XIII received respectively propolis (150 mg/kg), L-lysine (150 mg/kg) and water ig for 40 wks. After 40 wks, the surviving animals were anesthetized and subjected to femoral bone marrow aspiration and blood collection from the aorta, for CA and MNT, respectively, for investigation of genotoxicity. RESULTS: Groups IIB and K, which received only BBN and water, showed the greatest DNA damage in peripheral leukocytes (CA) and largest number of micronuclei in bone marrow erythrocytes (MNT) in relation to all other groups that received BBN and lysine and/or propolis (p<0.001). CONCLUSIONS: Both propolis and L-lysine are effective in protecting against genotoxicity, as well not being genotoxic themselves toward the cells evaluated, at the doses and times administered and according to the two tests utilized. .

Cardiovascular disease is one of the leading causes of death worldwide, and evidence indicates a correlation between the inflammatory process and cardiac dysfunction. Selective inhibitors of cyclooxygenase-2 (COX-2) enzyme are not recommended for long-term use because of potentially severe side effects to the heart. Considering this and the frequent prescribing of commercial celecoxib, the present study analyzed cellular and molecular effects of 1 and 10 µM celecoxib in a cell culture model. After a 24-h incubation, celecoxib reduced cell viability in a dose-dependent manner as also demonstrated in MTT assays. Furthermore, reverse transcription-polymerase chain reaction analysis showed that the drug modulated the expression level of genes related to death pathways, and Western blot analyses demonstrated a modulatory effect of the drug on COX-2 protein levels in cardiac cells. In addition, the results demonstrated a downregulation of prostaglandin E2 production by the cardiac cells incubated with celecoxib, in a dose-specific manner. These results are consistent with the decrease in cell viability and the presence of necrotic processes shown by Fourier transform infrared analysis, suggesting a direct correlation of prostanoids in cellular homeostasis and survival.

BACKGROUND: Novel, in silico-designed anticancer compounds were synthesized in our laboratory namely, 2-ethyl-3-O-sulphamoyl-estra-1,3,5(10),15-tetraen-17-ol (ESE-15-ol) and 2-ethyl-3-O-sulphamoyl-estra-1,3,5(10)16-tetraene (ESE-16). These compounds were designed to have improved bioavailability when compared to their source compound, 2-methoxyestradiol. This theoretically would be due to their increased binding affinity to carbonic anhydrase II, present in erythrocytes. Since the novel compounds under investigation are proposed to be transported within erythrocytes bound to carbonic anhydrase II, the morphological effect which they may exert on whole blood and erythrocytes is of great significance. A secondary outcome included revision of previously reported procedures for the handling of the whole blood sample. The purpose of this study was twofold. Firstly, the ultrastructural morphology of a healthy female's erythrocytes was examined via scanning electron microscopy (SEM) after exposure to the newly in silico-designed compounds. Morphology of erythrocytes following exposure to ESE-15-ol and ESE-16 for 3 minutes and 24 hours at 22°C were described with the use of SEM. The haemolytic activity of the compounds after 24 hours exposure were also determined with the ex vivo haemolysis assay. Secondly, storage conditions of the whole blood sample were investigated by determining morphological changes after a 24 hour storage period at 22°C and 37°C. RESULTS: No significant morphological changes were observed in the erythrocyte morphology after exposure to the novel anticancer compounds. Storage of the whole blood samples at 37°C for 24 hours resulted in visible morphological stress in the erythrocytes. Erythrocytes incubated at 22°C for 24 hours showed no structural deformity or distress. CONCLUSIONS: From this research the optimal temperature for ex vivo exposure of whole blood samples to ESE-15-ol and ESE-16 for 24 hours was determined to be 22°C. Data from this study revealed the potential of these compounds to be applied to ex vivo study techniques, since no damage occurred to erythrocytes ultrastructure under these conditions. As no structural changes were observed in erythrocytes exposed to ESE-15-ol and ESE-16, further ex vivo experiments will be conducted into the potential effects of these compounds on whole blood. Optimal incubation conditions up to 24 hours for whole blood were established as a secondary outcome.

BACKGROUND: During the last few years it has been shown in several laboratories that Celecoxib (Cx), a non-steroidal anti-inflammatory agent (NSAID) normally used for pain and arthritis, mediates antitumor and antiangiogenic effects. However, the effects of this drug on a tumor cell line resistant to chemotherapeutical drugs used in cancer have not been described. Herein we evaluate the angiogenic and antitumor effects of Cx in the development of a drug-resistant mammary adenocarcinoma tumor (TA3-MTXR). RESULTS: Cx reduces angiogenesis in the chick embryonic chorioallantoic membrane assay (CAM), inhibits the growth and microvascular density of the murine TA3-MTXR tumor, reduces microvascular density of tumor metastases, promotes apoptosis and reduces vascular endothelial growth factor (VEGF) production and cell proliferation in the tumor. CONCLUSION: The antiangiogenic and antitumor Cx effects correlate with its activity on other tumor cell lines, suggesting that Prostaglandins (PGs) and VEGF production are involved. These results open the possibility of using Celecoxib combined with other experimental therapies, ideally aiming to get synergic effects.

ABSTRACT

Hepatic steatosis is common in obese individuals with hyperinsulinemia and is an important hepatic manifestation of metabolic syndrome. Sterol regulatory binding protein-1c (SREBP-1c) is a master regulator of lipogenic gene expression in the liver. Hyperinsulinemia induces transcription of SREBP-1c via activation of liver X receptor (LXR) and specificity protein 1 (Sp1). Cilostazol is an antiplatelet agent that prevents atherosclerosis and decreases serum triglyceride levels. However, little is known about the effects of cilostazol on hepatic lipogenesis. Here, we examined the role of cilostazol in the regulation of SREBP-1c transcription in the liver. The effects of cilostazol on the expression of SREBP-1c and its target genes in response to insulin or an LXR agonist (T0901317) were examined using real-time RT-PCR and western blot analysis on cultured hepatocytes. To investigate the effect of cilostazol on SREBP-1c at the transcriptional level, transient transfection reporter assays and electrophoretic mobility shift assays (EMSAs) were performed. Cilostazol inhibited insulin-induced and LXR-agonist-induced expression of SREBP-1c and its downstream targets, acetyl-CoA carboxylase and fatty acid synthase, in cultured hepatocytes. Cilostazol also inhibited activation of the SREBP-1c promoter by insulin, T0901317 and Sp1 in a luciferase reporter assay. EMSA analysis showed that cilostazol inhibits SREBP-1c expression by repressing the binding of LXR and Sp1 to the promoter region. These results indicate that cilostazol inhibits insulin-induced hepatic SREBP-1c expression via the inhibition of LXR and Sp1 activity and that cilostazol is a negative regulator of hepatic lipogenesis.

ABSTRACT

The complex metabolic syndrome, diabetes mellitus, is a major human health concern in the world and is estimated to affect 300 million people by the year 2025. Several drugs such as sulfonylureas and biguanides are presently available to reduce hyperglycemia in diabetes mellitus. These drugs have side effects and thus searching for a new class of compounds is essential to overcome this problems. A series of seven novel N-[4-phenylthiazol-2-yl]benzenesulfonamides derivatives were synthesized and assayed in-vivo to investigate their antidiabetic activities by streptozotocin-induced model in rat. These derivatives showed considerable biological efficacy when compared to glibenclamide, a potent and well-known antidiabetic agent, as a reference drug. Four of the compounds were effective, amongst which 13 show more prominent activity at 100 mg/Kg p.o. The experimental results are statistically significant at p < 0.05 level

OBJECTIVE: Ischemic stroke may result from transient or permanent reductions of regional cerebral blood flow. Polymorphonuclear neutrophils have been described as the earliest inflammatory cells to arrive in ischemic tissue. CXCR1/2 receptors are involved in the recruitment of these cells. However, the contribution of these chemokine receptors during transient brain ischemia in mice remains poorly understood. In this work, we investigated the effects of reparixin, an allosteric antagonist of CXCR1/2 receptors, in a model of middle cerebral artery occlusion and reperfusion in mice. METHODS: C57BL/6J male mice treated with reparixin or vehicle were subjected to a middle cerebral artery occlusion procedure 1 h after the treatment. Ninety minutes after ischemia induction, the monofilament that prevented blood flow was removed. Twenty-four hours after the reperfusion procedure, behavioral changes, including motor signs, were analyzed with the SmithKline/Harwell/lmperial College/Royal Hospital/Phenotype Assessment (SHIRPA) battery. The animals were sacrificed, and brain tissue was removed for histological and biochemical analyses. Histological sections were stained with hematoxylin and eosin, neutrophil infiltration was estimated by myeloperoxidase activity and the inflammatory cytokine IL-iβ was measured by ELISA. RESULTS: Pre-treatment with reparixin reduced the motor deficits observed in this model of ischemia and reperfusion. Myeloperoxidase activity and IL-iβ were reduced in the reparixin-treated group. Histological analysis revealed that ischemic injury was also attenuated by reparixin pre-treatment. CONCLUSIONS: Our results suggest that the blockade of the CXCR1/2 receptors by reparixin promotes neuroprotective effects by reducing the levels of polymorphonuclear infiltration in the brain and the tissue damage associated with middle cerebral artery occlusion and reperfusion.

OBJETIVO: Avaliar a eficácia e conforto dos pacientes portadores de glaucoma primário de ângulo aberto (GPAA) ou hipertensão ocular (HO) em uso da combinação fixa de timolol 0,5% e brinzolamida 1%. MÉTODOS: Foram acompanhados prospectivamente 26 pacientes portadores de GPAA ou HO, num total de 50 olhos que foram instituídos a utilizarem a combinação fixa de timolol 0,5% e brinzolamida 1%. As avaliações foram feitas por um único examinador por tonometria de aplanação (Goldman) em 7 e 30 dias. Os possíveis efeitos colaterais e intolerância foram descritos pelos próprios pacientes através da pergunta: "Você sentiu alguma alteração ao pingar a medicação prescrita?" Os dados foram coletados e analisados estatisticamente. RESULTADOS: Os valores de pressão intraocular (PIO) foram significativamente menores nas avaliações de 7 e 30 dias (p<0,001). A média da redução da PIO foi de 38%, variando de uma média inicial de 23,8 mmHg para 14,6 e 14,4 mmHg nos dias 7 e 30, respectivamente. Dos 26 pacientes incluídos apenas 4 relataram alguma queixa ao pingar o colírio, sendo que as queixas variaram de ardência, leve queimação e embaçamento. CONCLUSÃO: A combinação fixa de timolol 0,5% e brinzolamida 1% mostrou-se eficaz no tratamento de pacientes com GPAA e HO com eficácia semelhante a da literatura e apresentou um baixo índice de efeitos desconfortáveis relatados pelos usuários da medicação.


OBJETICVE: To evaluate the efficacy and side effects of timolol 0,5% and brinzolamide 1% fixed combination in patients with primary open angle glaucoma (POAG) and ocular hypertension (OH). METHODS: 50 eyes of 26 patients with POAG or OH were evaluated with topical therapy with fixed combination of timolol 0.5% and brinzolamide 1%.The measurements with Goldmann tonometry were applied by only one ophthalmologist after 7 and 30 days on medication. The side effects were described by the patient based on the following question: "Did you feel any alteration with the prescribed drops?" The data were collected and analized statistically. RESULTS: The intraocular pressure (IOP) was lower in 7 and 30 days (p<0.001).The mean reduction in IOP was 38% with a variation from 23,8 mmhg to 14.6 and 14.4 mmhg in 7 and 30 days. Four patients had side effects: burning and blurring vision were related. CONCLUSION: the fixed combination of timolol 0.5% and brinzolamide 1% had good results with lower IOP in the treatment of patients with POAG and OH just like in the literature and had few side effects.

ABSTRACT

The relaxant effect of the methyl ester of rosuvastatin was evaluated on aortic rings from male Wistar rats (250-300 g, 6 rats for each experimental group) with and without endothelium precontracted with 1.0 µM phenylephrine. The methyl ester presented a slightly greater potency than rosuvastatin in relaxing aortic rings, with log IC50 values of -6.88 and -6.07 M, respectively. Unlike rosuvastatin, the effect of its methyl ester was endothelium-independent. Pretreatment with 10 µM indomethacin did not inhibit, and pretreatment with 1 mM mevalonate only modestly inhibited the relaxant effect of the methyl ester. Nω-nitro-L-arginine methyl ester (L-NAME, 10 µM), the selective nitric oxide-2 (NO-2) inhibitor 1400 W (10 µM), tetraethylammonium (TEA, 10 mM), and cycloheximide (10 µM) partially inhibited the relaxant effect of the methyl ester on endothelium-denuded aortic rings. However, the combination of TEA plus either L-NAME or cycloheximide completely inhibited the relaxant effect. Inducible NO synthase (NOS-2) was only present in endothelium-denuded aortic rings, as demonstrated by immunoblot with methyl ester-treated rings. In conclusion, whereas rosuvastatin was associated with a relaxant effect dependent on endothelium and hydroxymethylglutaryl coenzyme A reductase in rat aorta, the methyl ester of rosuvastatin exhibited an endothelium-independent and only slightly hydroxymethylglutaryl coenzyme A reductase-dependent relaxant effect. Both NO produced by NOS-2 and K+ channels are involved in the relaxant effect of the methyl ester of rosuvastatin.

ABSTRACT

Cigarette smoke (CS) is a rich source of radicals, predisposing the cell to oxidative stress resulting in inflammation. Chronic inflammation is a recognized risk factor for carcinogenesis. Cyclooxygenase-2 (COX-2) is a mediator of inflammatory pathway and may, therefore, contribute to carcinogenesis. There are several reports that suggest the association between CS and COX-2 associated risk to cancer. In the present study, we examined the role of celecoxib (a selective COX-2 inhibitor) in modulating the oxidative stress caused by CS inhalation in mice. CS exposure for a period of 10 weeks caused oxidative stress in the pulmonary and hepatic tissues, as evident from the increase in lipid peroxidation levels (LPO) and decrease in reduced glutathione (GSH) levels. Celecoxib (125 mg/kg body weight for 8 weeks) administration to CS inhaling mice reduced the oxidative stress by decreasing the LPO levels and enhancing the GSH levels in comparison to the CS-exposed group. CS exposure repressed the enzymatic antioxidant defense system, as evident from the decrease in catalase (CAT) and superoxide dismutase (SOD) activities. Co-adminstration of celecoxib considerably reversed the changes in the enzymatic antioxidant defense system. Histopathological studies of lungs showed that CS exposure induced alveolar wall destruction and air space enlargement. In co-treated group, the alveolar septa were thicker than normal with apparent infiltration of inflammatory cells. In CS-exposed group, hepatic tissue exhibited vacuolization and macrophage infiltration. Co-treatment with celecoxib restored the normal histoarchitechture in hepatic tissues of CS inhaling mice. Thus, the present study demonstrated that celecoxib adminstration reduced the oxidative stress-mediated risk to carcinogenesis, due to its ability to boost the antioxidant defense system.

ABSTRACT

The introduction of antiretroviral therapy in Trinidad and Tobago in the 1980s has resulted in a decrease in mortality of HIV-infected persons. Poor adherence to antiretroviral therapy (ART) has resulted in the development of multidrug resistant HIV. Resistance testing done on 40 samples showed that 64.8% of patients had K103 mutation, 75.6% of patients had M184 mutations and 62% of patients showed resistance to tenofovir suggesting that the K65R mutation was highly likely to be present. There was reduced activity to the protease inhibitors; no resistance was found to the protease inhibitor, darunavir. Thus, there is a need for salvage therapy to be introduced which will result in virologic suppression and potentially stop the spread of multidrug resistant HIV. Darunavir, a new generation protease inhibitor, is an essential part of salvage therapy and needs to be introduced into the national formulary.


La introducción de la terapia antiretroviral en Trinidad y Tobago en la década de 1980, ha producido una disminución en la mortalidad de personas infectadas por el VIH. La adhesión pobre a la terapia antiretroviral (TAR) ha conducido al desarrollo de una variedad de VIH resistente a las multidrogas. Las pruebas de resistencia realizadas a 40 muestras mostró que el 64.8% de los pacientes tenían mutación K103, 75.6% de los pacientes tenían mutaciones M184, y 62% de pacientes mostraron resistencia al tenofovir, lo que indica una alta probabilidad de mutación K65R. Había actividad reducida respecto a los inhibidores de la proteasa; mientras que no se halló ninguna resistencia en el inhibidor de la protease, darunavir. Así, hay necesidad de introducir una terapia de salvamento qué produzca una supresión virológica y potencialmente detenga la diseminación del VIH resistente a las multidrogas. El darunavir - inhibidor de nueva generación frente a la proteasa -es una parte fundamental de la terapia de salvamento y necesita ser introducido en el formulario nacional.

ABSTRACT

The present study was performed to determine the effect of intracerebroventricular [ICV] administration of W- 7, a specific calmodulin inhibitor, on the development of tolerance to antinociceptive effect morphine administration. This study was carried out on male wistar rats, weighing 200-250 g. Morphine was administered daily [15 mg/kg for 8 days]. The threshold to thermal nociceptive stimuli was measured by tail-flick test. W-7 [0.25, 0.5 and 1 micro mol/rat] was injected through ICV. Maximal possible effect percentage [MPE%] was considered as analgesia index. Our result showed that chronic morphine exposure induced tolerance to its antinociceptive effect and administration of W-7 [0.5 and 1 micro mol/rat] decreased the development of tolerance to it. In conclusion these data showed that chronic injection of W-7 inhibited the development of morphine tolerance which indicates that calmodulin and its dependent pathways may play a role in the morphine tolerance processes
